Protecting your privacy is important to United Dental Alliance ("UDA"). We are pleased to provide this Privacy Policy governing our collection and use of information.
Types of Information We Collect from Members In the course of providing or attempting to provide services to you, UDA may collect certain personal information about you. Personal information is information that identifies you individually ("Personal Information"), including your name, address, billing address, telephone numbers and/or electronic mail addresses. UDA may also collect information regarding our visitors that is not individually identifiable ("Non-Personal Information"), such as demographic information or Internet browser types.
How We Use the Information That We Collect from Members UDA uses your Personal Information to provide you with information about your account, to provide you the services you have requested, to improve the services UDA provides its members, and to inform you of additional products and services that we think may be of interest to you. UDA also uses this information and Non-Personal Information in other ways related to our services, including product development, operations, administration, and marketing. We do not share Personal Information with third parties except that we may store and disclose Personal Information as allowed or required by applicable law, including, without limitation, making disclosures (i) that are necessary or advisable to protect the rights, safety and property of UDA or others, (ii) that are necessary or advisable to conform to legal or regulatory requirements, (iii) to third parties in connection with the performance of services; (iv) as requested by law enforcement or other emergency agencies. UDA does not sell or lease Personal Information to anyone not affiliated with UDA.
UDA may use Non-Personal Information for various purposes, including but not limited to product development, member service, marketing and similar purposes. UDA may share Non-Personal Information with third parties.
UDA has confidentiality and security policies and procedures in place to help protect your Personal Information from unauthorized use or disclosure. All UDA employees are governed by these policies. UDA takes precautions to not identify you or your authorized representative when we receive an inquiry regarding your information. UDA also restricts access to your Personal Information to authorized employees and to certain third parties as described above. UDA disposes of your Personal Information and Non-Personal Information in accordance with all applicable laws.
Types of Information We Collect UDA does not collect Personal Information about you on the UDA Website unless you choose to provide that information to us or you choose a service on the UDA Website that requires you to submit personal information in order to receive the selected service. Personal Information gathered on UDA Websites may include, but is not limited to, your name, title, company, address, electronic mail address, phone number, etc. Unless specifically stated, we do not require Personal Information to obtain access to any part of the UDA Website.
"Cookies" Non-personal information and data may be automatically collected by the UDA Website through the use of "cookies." A "cookie" is a small amount of data transferred to your browser and read by the Web server that placed it there. Cookies are not programs that come onto a visitor's system and damage files. Generally, cookies work by assigning a unique number to the visitor that has no meaning outside the assigning site. The use of cookies is now standard operating procedure for most Websites. You can set your browser to notify you before you receive a cookie, giving you the chance to decide whether to accept it. You can also set your browser to turn off cookies. If you do so, however, some Web sites may not work properly.
How to set your cookie preferences:
Netscape Navigator In the Edit menu, select Preferences. In the Preferences dialog box, in the left hand side menu, click on "Advanced" and 4 options concerning cookies will appear on the lower right. Choose Help for further details. You can find more information about cookies and Netscape on the Netscape Web site.
Microsoft Internet Explorer MSIE allows you to set options for each of six privacy cookie settings: Block all Cookies; High; Medium high; Medium; Low; Accept all Cookies. For each of these settings, you can choose whether to accept, reject, or be prompted for all first and third party cookies, and to allow or not allow all session cookies. To set your cookie options, choose "Tools/Internet Options", and select the Privacy tab. Then, move the bar to one of the six cookie settings. Next, select the Advanced button, and click the Override Automatic Cookie Handling, and indicate the appropriate options.
For other browsers, contact the manufacturer of the browser for information.
User Name and Password Security If you have a user name and password for access to the non-public areas of the UDA Website, you are solely responsible for all activities that occur in connection with your user name and/or password. UDA recommends that you take steps to protect the confidentiality of your user name and password. If you become aware of any disclosure, loss, theft or unauthorized use of your user name and/or password, please notify UDA immediately.
Information You Submit Except as stated in the paragraphs above, any communication or material you transmit to the UDA by electronic mail or otherwise, including any data, questions, comments, suggestions, etc., is, and will be treated as, non-confidential and nonproprietary. Any such communication transmitted or posted to the UDA Website shall become the property of UDA and may be used for any purpose.
E-mail Communications If you provide your e-mail address to us, you consent to our use of your e-mail address to notify you of information that may affect your account, or to provide you with the information you have specifically requested. If you send us an e-mail message, you also consent to our use of your e-mail address to answer the message we received from you. Your e-mail address will not be used for any purpose other than those listed in this policy.
Linked Websites The UDA Website may contain links to other websites. These links are meant solely for your convenience. UDA has no control over, and is not responsible for, the information collection practices of the linked websites. Furthermore, your use of the linked websites will be governed by the privacy policies of those sites and not by this Privacy Policy. UDA recommends that you review the privacy policy of each of the linked website that you visit, before you provide them with any personal information.
Protecting Children's Privacy Online The UDA Website is not intentionally designed for or directed at children under the age of thirteen (13) years. It is our policy never to knowingly collect or maintain information about anyone under the age of thirteen (13).
California Residents California Civil Code Section 1798.83 permits members who are California residents and who have provided UDA with "personal information" (as that term is defined in Section 1798.83) to request certain information about the disclosure of that information to third parties for their direct marketing purposes. At the present time, UDA does not disclose "personal information" (as that term is defined in Section 1798.83) to any third party for that party's direct marketing purposes.
Changes UDA may revise this Privacy Policy at any time, without prior notice; however, the most current version of the Privacy Policy will always be available to you by clicking on the link at the bottom of the UDA Website.
